Medical Profile & Parental Permission for Medical Treatment
The Medical Profile & Parental Permission for Medical Treatment form has just been approved by Alpine Ski Nova Scotia for our use for all programs, dryland training, racing, travel and training camps. This form meets the criteria as it is set out in the Ski Club Risk Management Manual. This is extremely important information for the Wentworth Si Racing Club to have on file.
